4.2.2 Obtaining a Residential Worker's Consent to Care for a Looked After Child |
When a Child or Young Person is Accommodated, a representative of the Residential Unit or Home where the child/young person is placed must sign an agreement to take care of them
The Agreement Form is found in the Accommodate a Child Episode and is entitled:
The same form is used to obtain the following:
CLA: Agreements and Consent for Medical Treatment
The adult with Parental Responsibility’s consent to/request for the Child/Young Person to be accommodated (or the Young Person themselves if they are over 16 can consent to/request to be accommodated.
The adult with PR’s consent to Routine and Emergency Medical Treatment for the Child/Young Person.
The Friend or Relatives’ agreement to Look After the Child/Young Person.
The Foster Carer’s agreement to Look After the Child/Young Person.
It will be necessary to print off the consent and agreements form and then scan and upload it onto the episode once it has been signed.
It is not necessary for each signatory to sign the same document if that is not convenient. Just print off those sections that are relevant and scan and upload as many signed version as is necessary.
