9.20 Recording Personal Details on FWi

This guidance was added to the manual in September 2010.


When you are dealing with a child always make sure that the information on the Personal Details Screen is up to date. At point of referral or contact as much information should be recorded as possible and when, once a child is assigned or allocated to you, please check all the following:

Date of birth – is it correct? When a baby has been born make sure that the expected date of delivery has been confirmed as the date of birth.

UPN – all children of school age should have this.

Need Code – this should be entered at the point of referral and reviewed after each assessment or formal review such as CP conference, CiN Review, LAC Review.

Address – make sure the address is current, that it is where the child lives – particularly if now LAC or cared for by other adults than parents.

Phone numbers – ensure that these are up to date for the parent/carer and extended family members and professionals.

Gender – make sure this is correct/ updated particularly after a baby is born.

Ethnicity/ use of English, Language/Nationality - always make sure that these areas are as accurately recorded as FWi will allow, this includes both parents and updating when a baby is born.

Religion – Please get into the habit of asking parents whether they have religious beliefs or a faith and what they would want recorded for their child.

Health Visitor – Except in the early stages of pregnancy it should always be possible to identify a health visitor. If a health visitor’s details are not in the drop down menu please email Tara Egan or Tim Cloke with the name and practice/health centre and then update on personal details.

GP – With very rare exceptions you should be able to identify a GP for all children and adults. If the GP details are not present on the drop down menu please email Tara Egan or Tim Cloke with the name and practice/health centre and then update on personal details.

Other Involved Professionals – must be recorded on the front screen and not in case notes, reports or assessments.

Relationships – it is easy to assume that these do not change but they do. Please ensure they are updated, particularly after a child’s birth. Also, do not forget to create links to half-siblings, grandparents, aunts or uncles if already known on the system.

File Location – check this is up to date, particularly if there has been a recent transfer between teams or workers.

Care Packagesif you think a child should be allocated to you and it does not appear in care packages please advise your team manager immediately.
