This is a new chapter for October 2011.
When a Looked After Child/Young Person needs to move, a Change of Placement Episode must be opened. This will support the essential recording that is required. The Change of Placement Episode is accessed by the social worker closing down the Child's Plan and selecting Change of Placement which the social worker assigns to themselves
The Change of Placement episode contains:
- A Child’s Plan
- A CLA: Agreements and Consent for Medical Treatment Form
- A Placement Information Record
This documentation is the responsibility of the Child/Young Person’s allocated Social Worker however to save social work time any previous completed documentation will pull through for checking and amending if necessary.
The Child's Plan
Actions and Services to meet the Child/Young Person’s developmental needs should be considered by the Team Around the Child during the first week of the child/young person being Accommodated and should then be entered on the Child's Plan.
A CLA: Agreements and Consent for Medical Treatment Form
The consents will been completed before or at the point of a child/young person being accommodated and so do not have to be duplicated.
The appropriate agreement does need to be completed and signed by the carer for uploading onto FW
- Obtaining Parental Consent for a Child/Young Person to be Accommodated - No need to repeat
- Obtaining Parental Consents for Medical Treatment for an Accommodated Child/Young Person - No need to repeat
- Obtaining the Agreement of a Foster Carer to Care for a Looked After Child/Young Person - needs to be completed and signed if appropriate see Obtaining the Agreement of Foster Care to care for a Looked After Child
- Obtaining the Agreement of a Friend or Relative to Care for a Looked After Child/Young Person - needs to be completed and signed if appropriate see Obtaining the agreement of a Friend or Relative to care for a Looked After Child

A Placement Information Record
In order to support the best substitute care of the child/young person the carer needs as much information as possible from the social worker. The Placement Information Record should be completed as far as possible and presented to the carer.

Placement Planning Meeting
When a Child/Young person is accommodated a Placement Planning Meeting must be held within 3 days of the placement beginning.
In the event of the placement being with a Wandsworth Approved Foster Carer the responsibility for recording the Placement Plan is held by the Supervising Social Worker. In all other cases it is the responsibility of the allocated Social Worker
